Order the Admission Form for this Patient
These records can help you trace your family tree and understand the lives of your ancestors. Order a copy of the admission forms and register entry for £15.00. Watch our video to find out what you will receive.
For entries in this index after 1858 there is an entry for each patient in the ‘Notices of Admissions by the Superintendent of
the Mental Institutions’. For the research fee of £15.00 we will supply the entry from the ‘General Register’ and also locate the more detailed
admission forms for you. These forms are held by the National Records of Scotland in Edinburgh and are largely unindexed. You can view an
example of the admission forms.
As this involves a trip to the National Records of Scotland in Edinburgh it can take a few weeks to process your order. We usually try
to complete all orders within 3 weeks.

Order General Register Entry (only)
The information listed above is an index entry only, the full original record contains more information, including the date when the patient left
the institution or died. The research fee of £4.00 will include the provision of a digital image of the entry in the ‘General Register’ (NRS reference MC7)
relating to the patient, and we will also provide advice on whether more detailed but as yet unindexed records may be available and how these may
be accessed. Once you pay for your order we will conduct the research for you as soon as possible and e-mail the results to you, often within a
few hours (please note this may take longer at busy times or evenings/weekends). Find our more about what you will receive in our
Learning Zone.
